3. Valid NBI Clearance
4. Valid Certificate of No Marriage (CENOMAR) along with the receipt
5. College Diploma
6. Transcript of Records (TOR) or Certificate of Grades
7. Certificate of Eligibility
8. Whole Body Picture (4R size)
