2. Graduates of Senior High School Program.

3. Alternative Learning System Accreditation & Equivalency (ALS A&E) Test for Secondary Level Passer on 2016-2018, eligible for admission to college.

4. High school graduates of the old basic education curriculum.

𝐃𝐎𝐂𝐔𝐌𝐄𝐍𝐓𝐀𝐑𝐘 𝐑𝐄𝐐𝐔𝐈𝐑𝐄𝐌𝐄𝐍𝐓𝐒:

1. Certified True Copy of 𝟐𝐧𝐝 𝐨𝐫 𝟑𝐫𝐝 𝐐𝐮𝐚𝐫𝐭𝐞𝐫 𝐅𝐨𝐫𝐦-𝟏𝟑𝟖 (𝐑𝐞𝐩𝐨𝐫𝐭 𝐂𝐚𝐫𝐝) duly signed by the Class Adviser or Principal.

2. 𝐕𝐨𝐭𝐞𝐫’𝐬 𝐈𝐃 𝐨𝐫 𝐂𝐎𝐌𝐄𝐋𝐄𝐂 𝐂𝐞𝐫𝐭𝐢𝐟𝐢𝐜𝐚𝐭𝐢𝐨𝐧 that the Applicant is a registered voter in Laurel, Batangas. If the applicant is still not of legal voting age, Parent’s Voter’s ID should be presented.

3. 𝐁𝐚𝐫𝐚𝐧𝐠𝐚𝐲 𝐂𝐞𝐫𝐭𝐢𝐟𝐢𝐜𝐚𝐭𝐞 stating he/she is a resident of Municipality of Laurel for more than one (1) year.

4. Two (2) pcs. of 𝟏.𝟓” 𝐱 𝟏.𝟓” 𝐩𝐢𝐜𝐭𝐮𝐫𝐞 𝐰𝐢𝐭𝐡 𝐧𝐚𝐦𝐞 𝐭𝐚𝐠 (for application and test permit).

𝐈𝐌𝐏𝐎𝐑𝐓𝐀𝐍𝐓 𝐍𝐎𝐓𝐈𝐂𝐄:

*Applications with INCOMPLETE REQUIREMENTS will not be processed.

*The following are grounds for disqualification and nullification of application:

-Falsification/Misrepresentation of information entered in the application form

-Submission of incomplete and fraudulent documents.

-Late submission of required documents.

𝑹𝒆𝒎𝒊𝒏𝒅𝒆𝒓𝒔:

*High School graduates who have not enrolled immediately after they graduated who stopped for a year must present an Affidavit of Non-enrollment in technical or degree program to any other college or university.

*For applicants from school with non-numerical (Pass/Fail grades), please present a Certificate of Non-issuance of Numerical Grades (issued by the School Registrar).
